    The award is from the the Chicago Chapter of the National Academy of Recording Arts and Sciences. It consists of a wooden plaque with a bronze metal plate engraved with text.

    Patsy Montana was born Ruby Rose Blevins on October 30, 1908 in Beaudry, Arkansas and grew up near Hope, Arkansas. She was a singer and musician. She performed on various national radio shows including the popular, National Barn Dance”. She also appeared in several movies. Montana died May 3, 1996.
